I accept Highmark BCBS, UPMC, United and Optum insurances, as well as self-pay.  For out-of-network, I can provide you with a superbill to submit to your insurance company for reimbursement.

Cancellation Policy: Please note that if an appointment is missed or cancelled within less than 24 hours’ notice, the full appointment fee will be charged ($150).
